He underpromises and overdelivers, and always does what is in the best interest of the hair patient versus some other doctors who play games to increase their profit. Use the ladder shown in the image above to drop down to the ledge below. Be careful as there are a couple of traps placed close to the ladder. Ungroup your party members and use the character with the highest Dexterity to move down the ladder first. Start disarming the Peculiar Flowers before bringing your entire party to the level below.

Basilisk's Oil and a Potion of Vitality should do the trick here. At the start of this encounter, she will set Mayrina's cage on fire. There are several prisoners and victims in Ethel's lair. There's a dead, blinded victim, a petrified victim, and one in stasis with their head cut off.
How to Acquire Auntie Ethel's Hair
In Baldur's Gate 3, Auntie Ethel is one of the antagonists you'll encounter in different acts, and completing her quest line comes with various rewards. Certain NPCs related to Ethel's quest can be freed after defeating the hag, so here's a complete walkthrough of the Overgrown Tunnel in BG3. Find the small wooden door in the cavern (to the side of where you came in) and head in. Here, you'll end up in a workshop and it's worth looting as there are some useful potions and a handful of tasty camp supplies, too.
You'll encounter some Redcap NPCs as you continue onwards. They're harmless, but interesting- if you speak to them, one of them is pretending to be a sheep, showing that they were also part of the illusion surrounding this area. Continue onwards to the quest marker, following the path west. Auntie Ethel will confess that she does know Mayrina, and that she's staying at Auntie Ethel's house. Ethel made a vow not to reveal her location, which is why she didn't tell the brothers. Once you're done with the fight, you'll be able to loot the bodies.
Auntie Ethels hair… worth?
It shows you horrible visions of people who've tried to take down Ethel and manages to accidentally let slip that you can just walk through without any resistance. If you've been following our walkthrough closely, your party should level up somewhere around the swamp as you keep heading towards the quest marker. Spend some time in the Grove picking up new equipment and potions, and double check that you've got all the spells you want prepared. In particular, make sure you have water spells and/or water bottles. The best outcome in this scenario is to help Mayrina, as she is an important character you'll encounter once again in Act III. After saving Mayrina, she offered you a locket as a reward, but I suggest you refuse to take it.

Once that's done, she asks you to meet near the Riverside to help with Connor's burial. There is a waterfall illusion located in the eastern corner of this room. Check out the yellow arrow on the image above to see the precise location of this waterfall. After defeating Ethel inside the Gnarled Teahouse, walk towards the Fireplace (as shown in the image above). Walk through the Fireplace and climb down the stairs inside the small pathway to enter the Overgrown Tunnel.

You'll hear someone begging for help in the distance, but before you can get to them, Auntie Ethel will appear in her true form. She's a fey/hag, similar to a witch, and she informs you that this is her own 'personal playhouse' and that you need to leave or she'll eat you. Hair should be washed either the night before or morning of your style. Everyone receiving services on wedding days should come with clean 100% dry hair. If you do not meet the minimum in services you may still book at a rate of $935 + the cost of travel.
I was dreading fighting the victims non-lethally preciselly because I had my weapons poisoned, but they all became non-hostile soon as the dialogue ended. After traversing across to the other side, you'll enter a massive room with numerous poison traps disguised as Peculiar Flowers. Keep in mind these traps are deadly and can kill your entire party if not dealt with. Characters must go through a Perception check to identify and locate the traps. The best and easiest way to deal with these poison traps is to disarm the Vents located along the terrain. After saving Mayrina, enter through the Wooden Door to get inside the Acrid Workshop.
After disarming all the traps, you'll enter the Ancient Abode, where Mayrina is trapped inside a Gnarled Cage. Stand in front of the waterfall and aim at the ledge beyond the path. You can easily jump across the waterfall to reach the next area inside the Overgrown Tunnel.
This is the place where Auntie Ethel stores some of her magical items. I recommend picking up all of the essential items and reading through all the letters to understand Ethel's intentions. Once inside the Overgrown Tunnel, you'll encounter NPCs like Efrin, Lorin, Magron, and other characters trapped by Ethel. You can save Efrin and Lorin by speaking to them about the hag's death and instructing them to leave the Overgrown Tunnel safely. After informing Mayrina about her brothers, Ethel banishes her to a magical prison, and a fight ensues between the hag and your party. You'll also find Mayrina here, and you can inform her about Ethel's true intentions.
Take the open pathway located beside the table to enter the next section of the Overgrown Tunnel. Here, you encounter characters like Mask of Vengeance, Mask of Regret, Mask of Servitude, and Mask of Terrors. These characters won't be hostile to you, so you need not fight them. If you do end up fighting, make sure to steer clear of the muddy ground, as it reduces your movement speed drastically. I had most of my weapons coated with purple worm toxin (via the puddle dip method) and had Auntie with 5 hp when she offered the bargain. I got her to free Mayrina and let her go, but the hag straight up DIED after from the poison!
She killed Mayrina's brothers and is scheming to take Mayrina's child to gain a new hag body. Ethel also promised Mayrina that she would revive her dead husband (in a zombie form), so you'll have a decision to make towards the end of this quest regarding Connor. The Overgrown Tunnel is a secret passageway located inside the Gnarled Teahouse. The Riverside Teahouse Waypoint is the closest fast-travel point to Ethel's Teahouse. This is where you'll first encounter Auntie Ethel in her hag form.
Once you get Ethel's health low enough, she will stop the fight and ask to make you a deal. The sub-quest Save Mayrina will be added to your journal as her cage is set on fire. Once you get to the bottom of the path, save and heal, then press on. There's also a sentient gnarled door, which you're going to need to get through in order to progress here.
To save Mayrina, you must lower the Gnarled Cage and help her escape the Overgrown Tunnel. Start by walking towards the circular golden gimbal located in front of the Gnarled Cage. Interact with the Orb to set Mayrina free from the prison. Follow the roots pathway to descend towards the center of the room.

Auntie Ethel will speak to you, clearly shaken by the encounter. As you approach, you'll notice that the men are accusing Ethel of doing something to their sister. While Shadowheart will protest at the idea of you telling Auntie Ethel what's up with you, you'll need to tell her that you're infected with a Mind Flayer parasite. She is immediately concerned and sympathetic, then mentions that she might have something to help back home. To make finding the Riverside Teahouse easier, it's best to find Auntie Ethel in Emerald Grove after defeating the Goblins outside the gate during the start of Finding A Cure.
